At Healthevity, we specialize in Lifestyle Medicine and Obesity Medicine. We help you:

We focus on the six pillars of lifestyle medicine: nutrition, physical activity, sleep, stress management, social connection, and avoidance of risky substances.
We also use the latest in obesity medicine, including body composition analysis and tailored metabolic strategies. This is not a quick fix—it’s thoughtful, science-based health transformation.

We work with adults who want to improve their health through lifestyle and preventive care—especially those with:

•  Type 2 diabetes or prediabetes 
•  High blood pressure 
•  High cholesterol 
•  Overweight or obesity 
•  Other conditions that respond well to lifestyle changes

Your 60–90 minute visit includes:

•  Review of your health history, medications, and labs
•  Lifestyle assessment
•  Goal setting
•  A personalized care plan
•  A “health map” showing where you’ve been, where you are now, and where you want to go

When you join, you’ll receive:

•  Five 50-minute physician visits/year
•  Ongoing secure messaging between visits for plan adjustments and accountability.
•  My Viva Plan® subscription for nutrition, movement, and mindfulness
•  Monthly members-only group education/support via Zoom
•  Quarterly body composition scans
•  Access to a private resource library
•  Your own Healthevity notebook with custom handouts each visit

Your primary care provider is the central coordinator of your medical care, managing overall care, medications. preventive screenings, vaccinations, acute issues, and specialist referrals.

Healthevity is a consultative practice. We partner with you and your primary care provider to create a personalized plan rooted in lifestyle, behavior change, and whole-person care. Our role is to give you extended time, tailored strategies, and ongoing support that complements—not replaces—your primary care.

Healthevity Lifestyle and Integrative Medicine operates primarily as a cash-based, membership practice. Payment to the practice is accepted from HSA/FSA accounts and by credit or debit card.

To devote the greatest amount of attention and energy to patient care, and minimize the administrative burden related to claim processing, I have opted out of Medicare.

For the same reason, I am not a participating provider with any private medical insurance provider. While privately insured, non-Medicare recipients may request a superbill to submit to their private insurance carrier, our clinic can not guarantee coverage or reimbursement. Please note that our office is unable to participate in filing, negotiating or resolving any medical insurance claims. 

We recommend consulting your insurance provider to understand your terms of coverage before a medical appointment, particularly if you intend to submit a claim for compensation.

Yes this is $415 per visit*

Visits beyond those included in the membership are billed separately. Scheduling and billing for these extra visits is negotiated directly with the provider.

*In very special circumstances, at the discretion of the provider, individuals may be accepted outside of the membership plan and qualify for the per visit price.
