THE FOUNDATIONAL ASSESSMENT                                        $495 one time

An initial, one time, in-depth, 90-minute visit which includes:

 

• Conversational visit in a relaxed setting about your past family, social and medical history and any related factors.

 

• Extensive review and analysis of your medical records from other providers and detailed questionnaires you complete online before your visit.

 

• Comprehensive, detailed physical examination and a review of lab studies that are ordered, performed and received prior to your visit.

 

• Body composition analysis that goes beyond BMI  using bioimpedance technology.

 

• Creation of a personalized health road map based on the review of your health history, an assessment of your current status and a discussion about what you want to achieve along your wellness journey.

 

• Immediate recommendations to enhance your health based on a summary of all your health information, your goals, and your preferences about where to start.

 

Excluded from the above price is the cost of recommended laboratory studies that are performed and reviewed before your visit. Payment for the Foundational Assessment is made at the end of the discovery call to secure a reservation.  Once our office has had an opportunity to review medical records from other providers, lab results from any studies ordered and questionnaires and forms returned by you via the secure patient portal, we will contact you to schedule an appointment.

After the Foundational Assessment, if mutually agreeable, there will be opportunity to join the Healthevity Membership Plan. During this year long program, we will collaborate to transform your preliminary roadmap into specific, measurable, actionable, realistic and timely goals. Most importantly, we will identify the steps you're prepared to take, adapt them as needed, and ensure you have the support and resources essential for success.

 

THE HEALTHEVITY MEMBERSHIP PLAN                    $367 monthly/12 mos.

Yearlong program includes:

 

• Five, 50 minutes, in person, physician visits

 

• Customized prescriptions for optimal nutrition, physical activity, sleep and stress management that address your current needs and are modified as needed.

 

• Free access to My Viva Plan (myvivaplan.com), a digital health platform and app that allows you to customize and track meals, physical activity and mindfulness practices.

 

• Text messaging with Dr. Spaulding personally (not staff) for brief non-emergent questions and clarifications using the online patient portal or a secure app.

 

• Monthly Zoom seminars/educational sessions featuring group discussions and support with private access to on-demand recordings afterwards.

 

• Body composition analysis quarterly that goes beyond BMI  to objectively assess body fat and lean body mass.

 

• Private access to the Healthevity library of curated resources online including handouts, videos and other learning materials.

 

• Appointment scheduling online offered as a time saving feature.

 

• Convenient, secure access to your medical information, notes and plans via the patient portal and/or a separate patient app.

 

• Automatic monthly billing, payable by credit or debit card

 

ADDITIONAL VISITS                                                        $415 per visit*

Visits beyond those included in the membership are billed separately. Scheduling and billing for these extra visits is negotiated directly with the provider.

*In very special circumstances, at the discretion of the provider, individuals may be accepted outside of the membership plan and qualify for the per visit price.

 

PAYMENT AND INSURANCE POLICY

Healthevity Lifestyle and Integrative Medicine operates primarily as a cash-based, membership practice. Payment to the practice is accepted from HSA/FSA accounts and by credit or debit card.

To devote the greatest amount of attention and energy to patient care, and minimize the administrative burden related to claim processing, I have opted out of Medicare.

For the same reason, I am not a participating provider with any private medical insurance provider. While privately insured, non-Medicare recipients may request a superbill to submit to their private insurance carrier, our clinic can not guarantee coverage or reimbursement. Please note that our office is unable to participate in filing, negotiating or resolving any medical insurance claims. 

We recommend consulting your insurance provider to understand your terms of coverage before a medical appointment, particularly if you intend to submit a claim for compensation.
